what to do
NMB has developed a donor qualification process which is as convenient as possible while ensuring milk safety.

What is involved:
  1. Contact the National Milk Bank via email or phone
  2. The Donation Coordinator will send a welcome packet which includes the Donor Qualification Guidelines and Donor Consent Form.
  3. Once you have reviewed and signed the Consent Form, return it to NMB via fax, email, or postal service.
  4. Complete the Intake and Medical History Survey included in the welcome packet (also available via phone interview).
  5. If there are any questionable responses, the Medical Director will be consulted to confirm your eligibility.
  6. Once you have passed the survey, send the Medical Consent forms (included in the welcome packet) to your Physician and child's Pediatrician for completion and submittal. These forms ensure that you and your baby are not chronically ill.
  7. After all of above steps are completed, the Donation Coordinator will issue you a
    Donor Test Kit.
  8. NMB will arrange for a traveling phlebotomist to come to your residence to perform a blood draw and DNA cheek swab test. The sample will be sent to a blood lab for processing.
  9. Once passing results are confirmed, the Donation Coordinator will ship you materials and equipment to begin donating milk:
    • Thermometer for your freezer
    • Electric Breast Pump
    • Collection containers and preprinted barcode labels
    • Shipping containers and labels
    • Instructions for packing and shipping
  10. If you have previously stored milk, we may be able to accept it. Contact the Donation Coordinator for details.
